What is an ICF/MR? 
In 1972 the ICF/MR program was developed by Medicaid as an incentive to upgrade the quality of care in public institutions.  In order to receive funding through the ICF/MR program, facilities must ensure that residents receive the following services:  24 hour nursing care, access to education, day programming for adults, therapy services, and timely access to medical care.  The ultimate goal of an ICF/MR is to assist our residents in maintaining or achieving the highest level of independence.
 
 
How is Brookside Accredited? 
Brookside is licensed through the Department of Developmental Disabilities (DDD). Brookside is also a certified provider through the (Federal) Center for Medicaid Services (CMS).  The local monitoring authority for CMS is the Ohio Department of Health (ODH).
 
 
What are your placement criteria? 
Individuals seeking placement at Brookside must have a diagnosis of Mental Retardation.  Brookside specializes in the care of individuals with severe to profound mental retardation.  If interested in placement at Brookside, please contact our Social Worker for more information.
 
 
How is Brookside Funded? 
Brookside is primarily funded by Medicaid; however, if an individual does not qualify for Medicaid, private pay is also accepted.
 
 
What services does Brookside provide? 
Brookside provides the following services:   
  • 24-hour nursing care
  • physical therapy
  • occupational therapy
  • speech therapy
  • a licensed dietitian
  • therapeutic recreation
  • an aquatics program
  • psychological services
  • behavior intervention
  • social services
  • access to school and vocational programming
  • home-based training
  • in-house dental, optical, and podiatry services
  • transportation to and from medical appointments
  • sensory stimulation programs
 
What is "Active Treatment"? 
As an ICF/MR it is our goal to maintain the skills a person has when they come to Brookside and build upon those skills.  Active treatment is the process of taking every day situations and using them as learning opportunities.  For example, if a person is able to cut their food with a knife, we want to encourage the person to cut their food with a knife every time the opportunity presents itself.  If a person possesses a skill, the staff at Brookside will encourage the person to use their skill rather than stepping in and completing the task for the person.
 
 
What is your visitation policy? 
Family and friends are encouraged to visit as often as they would like.  Visiting hours are 8 a.m. - 8 p.m., Sunday through Saturday; however, accommodations can be made for families requesting visits outside of our regular visiting hours.  For security reasons, we ask that all visitors enter and exit our building through the front door.
 
 
Who handles medical appointments? 
Brookside's nursing department is responsible for scheduling and transporting residents to and from medical appointments.  Guardians are welcome to attend appointments if desired.  Many of our healthcare providers such as our dentist, optometrist, and podiatrist provide on-site services.  Brookside's Medical Director provides annual physicals and regular check-ups on-site as well.
 
 
What do residents do during the day?
Residents age 22 and older attend The Creative Learning Workshop which is located less than 2 miles from Brookside.  The Creative Learning Workshop provides the opportunity to learn new skills in a fun environment.  In addition to recreational and leisure programs, CLW has a job coach on site to assist with vocational programming.  School age residents, ages 22 and  under, are enrolled in Mason City Schools and offered modified educational services within specialized classrooms.
